This Mid-term Exam must be written under teacher supervision.

If you have a Distance Learning teacher/facilitator, ask him/her to contact your Spanish 30 teacher a few days prior to the exam. If you are arranging for a different teacher to supervise, please have him/her fill out the attached form. You may also make an appointment at the closest ADLC Campus to do your exam.

If they have not already done so, please have your supervisor fill out the attached On-line Declaration form and return it to ADLC. When you are ready to write the exam, have your supervisor contact the ADLC Spanish instructor, to have the exam released. Call your teacher if you need help or alternatives in making these arrangements.
