Lesson 17 — Activity 2: Completing an Application for a bank account
When you have a job, and even before you get a job, you may want to apply to open a bank account to manage your money.
As you saw with the SIN application form, you need to know many things to fill out an application form accurately. The same is true when you apply to open a bank account.
This activity looks at an example of a typical application form from a bank. It guides you in filling out a sample form.

Here are the sections of the application form for a bank account. Begin with Section 1.
The first section of the form helps the bank to know why you want to open your account with this particular bank. It also lets the bank know if anyone else will be sharing the account with you.
- Tell us why you chose ______ Bank.
- I chose ____ Bank because — Here, give the reason you chose the bank, such as it is close to where you work, it was recommended to you for good service, etc.
- I presently/previously banked with — Name of Financial Institution — This is where you fill in the name of any other bank you may deal with or have dealt with and — Branch Address — the mailing address of that bank.
- Will this account be used by or on behalf of a third party? — If you are the only person using the account, answer "No"; if someone else will be using the account with you, answer "Yes."
- Third Party's Name — Fill in the name of the person you are sharing the account with.
- Third Party's Address — Fill in the address of the person you are sharing the account with.
- Third Party's Principal Business or Occupation — Fill in the job of the person you are sharing the account with.
- Relationship to Account Holder — Show how you and the person you are sharing the account with are related (parent, spouse, sibling, etc.).
Section 2 of the form lets the bank know more about your identity and how to contact you.

- Mr. - Mrs. - Ms. - Miss - Other — Check the box that applies to you.
- Full Legal Name — Fill in your name (no nickname) in the order shown. First Name — Middle Name — Last Name
- Date of Birth — Fill in your birthday in the order shown with the Year first, then the Month (use either the name or the number such as January = 01, February = 02, and so on) and the Day.
- Social Insurance Number — Fill in the nine digits in the order they are on your card, one for each blank on the form.
- Home Address — Fill in your mailing address, followed by the name of your City or Town, your Province, and your Postal Code.
- E-mail Address (optional) — If you want the bank to have your e-mail address, you can fill this in, but you do not have to.
- Home Telephone and Cell Telephone — Fill in the phone number with the area code first in the brackets: (780) for areas in Alberta north of Ponoka, (403) for areas from Ponoka south. You do not have to include a cell phone number if you do not have one or if you would rather not give it to the bank.
- Mandatory Identification (Piece 1) and Mandatory Identification (Piece 2) — Fill in the types of identification you will show to prove that you are who you say you are. Commonly accepted pieces of identification are your driver's licence, birth certificate, or Certificate of Canadian Citizenship. Various banks might accept various proofs of identification.
- Place of Issue — Fill in where each piece of identification is from.
- Expiry Date — Fill in when each piece of identification expires or has to be renewed.
- Previous Address (if less than 2 years at present address) — Fill in the mailing address of the last place you lived if you have not been living at your current address for at least 2 years; include the City, Province, and Postal Code.
- Length of time at current address — Tell how long you have been living at the place you are living now.
- Length of time at previous address — If you have not been living at your current address for at least 2 years, tell how long you lived at the last place you lived.
Section 3 of the form tells the bank about your work situation.
YOUR EMPLOYMENT INFORMATION
- Self-Employed — Check Yes if you own your own business and No if you work for someone else.
- Employer/Business Name — Fill in the name of your boss or the business you work for.
- Business Telephone — Fill in the phone number where you work, beginning with the area code in brackets.
- Business Address — Fill in the mailing address of the place you work, followed by the City, Province, and Postal Code.
- Occupation — Fill in the name of the job you do.
- Length of time at present job — Fill in the amount of time you have been working at your job.
- Previous Employer (if less than 2 years at present) — Fill in the name of your last boss if you worked at a different job in the past 2 years.
- Business Telephone — Fill in the phone number where you worked before, beginning with the area code in brackets.
- Business Address — Fill in the mailing address of the place you worked before, followed by the City.
- Occupation — Fill in the name of the job you did.
- For Students — Fill in this section if you are still in school.
- Name and Address of Educational Institution — Give the name and address of the place you go to school.
- Full time — Check here if you go to school with a maximum course load. Part time — Check here if you are not taking as many classes as you could.
- Elementary - High School - University/College - Other — Check the level of school you are in.
- Year you are expecting to graduate 20 _ _ — Fill in the last numbers of the year you should finish school.
The rest of the form is for the people at the bank to fill in information, so we don't need to include anything else.
