Lesson 17 β€” Activity 3: Completing a Job Application Form


When you have your SIN and bank account (or even before), you may find yourself completing numerous application forms as you look for a job.

Application forms tell employers a lot about the people looking for jobs. If you fill out an application form with incorrect information or do a very sloppy job, the employers might think you would treat your work the same way. They might be much less likely to hire you than if you filled out the form neatly and correctly. It is usually a good idea to take two copies of an application form or to make a photocopy. That way, you can practise on the first one and complete the second one when you are sure you have all the correct information you need.

This activity looks at an example of a typical application form for a job. You will then try filling out a sample form for the assignment.

Here are the sections of the job application form. Begin with Section 1.
The first section of the form helps the employer to know some details about who you are and how to reach you.

PERSONAL INFORMATION

  • Date β€” Fill in the date of the day you are completing the form. Try to return the form to the business soon after completing the form so you look interested in getting a job.

  • Surname β€” Fill in your last name here, then your First Name and Middle Initial.

  • Phone Number β€” Fill in your phone number with the area code in brackets: (780) for areas in Alberta north of Ponoka, (403) for areas from Ponoka south.

  • Street Address β€” Fill in your mailing address followed by the number of your Apartment, if you live in one.

  • City β€” Fill in the name of the community where you live, followed by the Province and Postal Code.
The second section of the form lets the employer know whether you have any experience with the company and whether you would be able to work when needed.

APPLICATION INFORMATION


  • Position Applied For β€” Fill in the type of work you want to do, such as cashier, janitor, etc.

  • Date Available to Start β€” Fill in the first day you would be able to work (include the month, too).

  • Salary Expectations β€” Fill in the amount you would like to earn per hour. ($ /hour) Ask around to make sure that you are suggesting a reasonable amount that is not too low or too high.

  • Are You Legally Entitled to Work in Canada? β€” If you have your SIN, the answer is "Yes."

  • Have You Ever Worked at a _____ Before? β€” The employer wants to know if you have experience at this type of job and can also find out whether you were a good worker.

  • Location β€” Say where the business was if you did work at one before.

  • Store Number β€” Give the store number of the location where you worked before, if this applies.

  • Reason For Leaving? β€” Say why you quit the job the last time. Try to say it in a positive way, but do not lie about your reasons for leaving.

  • How Did You Find Out About This Opportunity? β€” Say where you heard about the job (in the paper, from a friend, etc.)

  • Hours Available β€” For the Days of The Week, show what time you could start work (From) and when you would have to stop (To). Be honest, or you might have trouble later if you get the job and want to work more or less than you indicated on the application.
  
 
The third section of the form tells the employer about what grade you are in in school.

EDUCATION

  • Institution β€” Under this heading, fill in the name of the place you go to school under the type of school it is: High School, College, University, Trade School, Other

  • Highest Grade Completed/ Number of Years Attended β€” Fill in the last grade you finished.

  • Diploma/ Degree β€” Fill in the name of the diploma you received if you finished school.  
 
The last main section of the form tells the employer what kind of jobs you have had before. If you haven't worked before, you wouldn't fill in this section.

EMPLOYMENT HISTORY
(Starting with most recent) β€”  This means that you give first the information for the job you have now or the job you worked at last, then, the one you had before that, and so on into the past.

  • Current Employer Name and Address β€” Give the name and mailing address of the place you are working now. If you are not working now, do not fill this in.

  • Job Title β€” Fill in the name of your job.
 
  • Describe Duties β€” Tell the employer about the things you do at your job.

  • Date (Month & Year) β€” From β€” Fill in the month and year you began the job. Toβ€”  Fill in the month and year it is now.

  • Supervisor's Name  β€” Fill in the name of the person you report to.

  • Rate of Pay  β€” Fill in the amount you get paid  per hour.

  • Phone Number β€” Fill in the phone number of the place you work.

  • Reason for Leaving β€” Explain in a positive way why you are leaving that job.

  • Previous Employer Name and Address β€” Fill in the name and mailing address of the place you worked before the job described above. If you did not have a job before, do not fill this in. Fill in the rest of the information as suggested above.

  • Second Previous Employer Name and Address β€” Fill in the name and mailing address of the place you worked before the job described above. If you did not have a job before, do not fill this in. Fill in the rest of the information as suggested above. 
 
   

REFERENCES β€” You should call previous employers to let them know that you are applying for a job and that they might get a call from the places you are applying.

ACTIVITIES β€” This is a good place to let the employer know about any special things you do that will show that you are an enthusiastic, active person.

SIGNATURE β€” Sign as you usually sign your name. DATE β€” Try to return the form close to the date you sign it so the employer does not think that you are forgetful or lazy!
