HEALTH AND WELLNESS E-PORTFOLIO
This assignment is made up of two parts. Be sure to submit both items. There are 2 submission pages that follow, in which you will submit the items outlined below.
Unit 2 Deep Dive asked you to complete a Wellness Inventory by taking an email survey or by responding to the questions in an interactive PDF link. You will now upload this completed inventory as Part A of this assignment.
If you did not complete this Inventory at the start of the unit, you can choose ONE of the following options to complete and submit;

Balancing the Dimensions - Assignment and Marking Guide
In order to be truly healthy, you need to balance all five dimensions. You will find you have strengths in some dimensions and room for improvement in others. For example; you may spend 60 minutes a day being active, but if you are isolating yourself from all your friends and family your overall health is going to be affected. Dimensions over-lap and It is important to maintain harmony throughout the dimensions and not focus on just one area. The goal is to balance all dimensions of wellness.
You will complete the following 3 steps using one of the templates provided below. Submit the template upon completion.
Step 1:
1. Describe a time when your dimensions of health were unbalanced. Which dimensions of health were neglected and how did this affect you?
2. What did you do or could have done to improve the situation?
3. What was the outcome?
4. How did this affect your future wellness goals? Be specific and provide details.
Step 2:
For each dimension of health, create a new challenge that would improve your over-all wellness. Ensure these are different from the Health Challenges stated in the course.
Step 3;
Choose one of your newly created challenges and outline what steps you need to take to achieve your goals.
